Job Description/Overview:
As a Care & Support Worker, you will support residents with enduring mental health issues, promoting their independence and emotional well-being. The role involves working with residents in self-contained flats, managing support plans, and encouraging active community engagement.
Key Responsibilities:
Empower customers to establish and maintain positive relationships in the community.
Provide practical support for daily living tasks.
Promote independence and emotional well-being.
Monitor and respond to changes in customer behavior and safeguarding issues.
Collaborate with colleagues, external agencies, and family members to provide cohesive care.
Maintain accurate customer records and administer medication as per care plans.
De-escalate challenging behaviors respectfully and professionally.
General Responsibilities:
Comply with safeguarding, safety, and compliance policies.
Promote MTVH’s values of Care, Dare, and Collaborate.
Maintain confidentiality and adhere to Health & Safety policies.
Qualifications:
Level 1 Health & Social Care qualification or willingness to work towards it.
Experience:
Previous experience working with vulnerable adults.
Skills:
Basic IT literacy.
Strong verbal communication skills.
Ability to read, write, and understand basic English.
