Job Description/Overview:
East and North Hertfordshire NHS Trust is recruiting a Personalised Care Support Worker/Administrator for the Breast Cancer team. This role involves coordinating personalised care pathways, managing follow-up appointments, and providing administrative support for the Breast Unit. The ideal candidate will have excellent IT and communication skills and work alongside the multidisciplinary team to ensure exceptional patient care.
Key Responsibilities:
Act as the main point of contact for scheduling patient appointments and follow-up pathways.
Triage patient communications, signposting them to the appropriate team member.
Maintain and manage the PCFU patient database and coordinate MDT reviews.
Assist in organising survivorship workshops, including agendas and patient attendance.
Monitor patient needs, report changes to the MDT, and provide timely administrative support.
Enter patient data accurately and securely into hospital systems.
Track and manage health records in accordance with departmental policies.
Support the Breast Unit with general administrative tasks.
Qualifications:
Good General Education.
NVQ Level 4 in health or social care or equivalent experience.
Desirable Qualifications:
Further study in healthcare or cancer care.
Teaching or IT qualifications.
Experience:
Previous experience in administration and data entry.
Experience dealing with challenging situations.
Desirable Experience:
Experience with NHS systems (e.g., EPR, Infoflex).
Formal/informal teaching experience or initiating change.
Skills:
Strong IT skills, including Microsoft Office.
Ability to work independently and as part of a team.
Excellent communication and organizational skills.

Additional Information:
Salary: £26,530 – £29,114 per annum plus 5% HCAS.
