Job Description/Overview:
We are seeking an experienced administrator with strong communication and IT skills to support our Colorectal Cancer teams in implementing personalized care pathways. The Personalised Care Support Worker will be a key member of the Colorectal multidisciplinary team, working alongside Clinical Nurse Specialists (CNS) to address the physical, social, and psychological needs of patients post-cancer treatment. Responsibilities include coordinating patient appointments, managing PCFU databases, and providing emotional support to patients.
Key Responsibilities:
Coordinate patient appointments and follow-up pathways.
Act as the first point of contact for patient queries, signposting to appropriate team members.
Maintain the PCFU database, collaborating with the MDT coordinator.
Monitor patient needs and report condition changes to the MDT.
Support clinical staff by identifying patients in need of a Holistic Needs Assessment (HNA).
Qualifications:
Good general education.
NVQ Level 4 in health or social care or equivalent experience.
Experience:
Essential: Data entry, administration, and handling challenging situations.
Desirable: NHS experience, familiarity with Hospital IT systems, and experience in Clinical Governance.
Skills:
Strong IT skills, especially Microsoft Office.
Effective communication and interpersonal skills.
Ability to work both independently and collaboratively within a multidisciplinary team.
