Job Description/Overview:
The Social Worker will engage with residents and families, conducting assessments, counseling, and providing support in areas such as social needs, financial assistance, and community resources. Responsibilities include managing outreach programs, coordinating discharge services, monitoring family involvement, and offering leadership in interdisciplinary meetings. The role includes oversight of a part-time social worker’s work quality and documentation.
Key Responsibilities:
Interview residents and families to identify social work needs, including a social history assessment.
Provide individual counseling and lead group activities.
Assist residents and families in understanding administrative forms and financial assistance options.
Coordinate discharge planning and advocate for residents’ needs.
Guide residents and families in accessing community services.
Maintain records and provide clinical social work as necessary.
Monitor family involvement, outreach as needed, and lead interdisciplinary team meetings.
Offer sensitivity and rights training to staff, along with specialized training as required.
Qualifications:
Bachelor’s degree in Social Work or related field is preferred.
Experience:
Relevant experience in social work, particularly in a healthcare or community support setting.
Skills:
Strong counseling and advocacy skills.
Excellent organizational and documentation abilities.
Leadership skills for interdisciplinary meetings and staff training.
