Job Description/Overview:

Key Responsibilities:
Conduct psychosocial assessments and provide counseling to patients and families
Assist with the emotional and social needs of patients in hospice care
Collaborate with the healthcare team to develop and implement care plans
Provide support and resources to families during the end-of-life process
Ensure continuity of care and advocate for patients’ rights and needs
Qualifications:
Licensed Medical Social Worker (LMSW) required
Experience in hospice or palliative care preferred
