Job Description/Overview:
The Social Worker at Trinity Health Michigan will assist patients and families in coping with severe and long-term illnesses. Responsibilities include conducting interviews, preparing psychosocial assessments, developing treatment plans, providing counseling, and directing patients to community resources. The role also includes collaboration with care teams, providing consultation, and developing patient care programs.
Key Responsibilities:
Conduct patient and family interviews and prepare psychosocial assessments
Develop treatment plans and provide direct services such as counseling
Coordinate care for high-risk patients and refer them to community resources
Participate in interdisciplinary care management teams
Advocate for patient rights and support patients in navigating legal and ethical issues
Document assessments and progress notes
Contribute to quality improvement processes and patient care programs
Provide leadership in related committees and workgroups
Qualifications:
Master’s degree in social work or psychology from an accredited institution
Current State of Michigan master’s social worker license or limited social worker license
Six to twelve months of related experience preferred
Experience:
6-12 months of related experience preferred
Skills:
Strong interpersonal and analytical skills
Ability to handle detailed work and manage multiple tasks
Ability to work effectively within a multidisciplinary team
Strong clinical competence in providing interventions at individual, family, and group levels
