Job Description/Overview:
Waterview Health and Rehab is seeking a compassionate and skilled Assistant Social Worker to join our team. The Social Worker Assistant plays a crucial role in supporting the social and emotional well-being of residents and their families. This role involves actively assisting with the pre-admission and admissions processes and providing individual and group help when necessary.
Key Responsibilities:
Assist with pre-admission and admissions processes.
Provide compassionate support to residents and their families.
Maintain and improve residents’ ability to manage their physical, mental, and psychosocial needs.
Initiate interventions for residents facing mental and behavioral health challenges.
Develop and maintain individualized care plans.
Participate in interdisciplinary care plan meetings to address residents’ social and emotional needs.
Collaborate with the finance team to ensure proper documentation and resident education.
Coordinate discharge planning and arrange necessary home care or equipment for residents.
Qualifications:
Associate Degree in healthcare (preferred but not required).
Strong multitasking and communication skills.
Experience in long-term care is a plus.
Experience:
Previous experience in a healthcare or social work setting preferred.
Skills:
Strong ability to manage tasks and communicate effectively.
