You will oversee the effective monitoring, coordination, and implementation of care policies and procedures, ensuring compliance with care legislation, company policies, and best practices.
Key Responsibilities
Develop and implement outcome-focused personal care plans.
Conduct reviews and compile detailed, accurate reports and action plans within specified timelines.
Ensure compliance with relevant legislation and good care practices.
Build strong relationships with service users, their families, and community partnerships.
Support the digitalization of social care processes, particularly in care planning and service reviews.
Monitor and maintain high standards of service delivery across staff and support workers.
Deliver emergency care if required and support out-of-hours care on a rota basis.
Qualifications
Experience working with older people or in a care setting.
Strong knowledge of the care sector and person-centered approaches.
Familiarity with care legislation, care planning, and digital care solutions.
Proficiency in Microsoft Office.
Experience
Prior experience in creating care plans and managing Self-Directed Support (SDS) packages.
Skills
Excellent communication and customer service skills.
Ability to build and maintain strong relationships.
Strong organizational skills and attention to detail.
Positive attitude towards achieving high standards in service delivery.
