Position:Ward Coordinator

Job Description/Overview:
The Ward Coordinator will assist in maintaining efficient patient flow in the hospital by liaising with the medical and nursing teams. You will be responsible for coordinating patient referrals, assessments, and admissions, as well as working with the discharge team to ensure safe and timely discharges. This role supports patient safety and timely treatment.
Key Responsibilities:
Act as the first point of contact for communication on the ward.
Ensure discharge/care plans are current and up to date.
Provide clerical and general support to the ward/departmental team.
Assist in the safe discharge of patients, including liaising with medical teams and discharge team.
Assist with meals and patient beverages as required.
Qualifications:
Essential: General education.
Desirable: NVQ in Health and Social Care.
Experience:
Essential: Experience in an acute care environment, experience working in a multidisciplinary team, excellent written and verbal communication skills, and basic IT skills.
Desirable: Experience with patient admissions/discharges, risk assessments, and referrals process.
Skills:
Strong communication and organizational skills.
Ability to manage own workload.
Proficiency in basic IT.
