Position: Healthcare Assistant – Outpatients

Job Description/Overview:
The NHS is evolving with a stronger focus on community-based care, creating a great time to join our team. We are seeking compassionate Healthcare Assistants dedicated to patient-centered care, focusing on holistic rehabilitation. As part of a nurse-led team across two hospitals, you will support patients who need assistance transitioning back home or nearing end-of-life care. This role requires independent decision-making under the direction of the nurse in charge.
Key Responsibilities:
Provide essential in-patient care as part of a community hospital setting.
Collaborate within a team to support local patients and families.
Monitor and maintain high standards of care.
Implement and support individualized care plans for a positive patient experience.
Qualifications:
Relevant Level 2 qualification (NVQ/QCF certificate in care or equivalent).
Functional Skills Level 2 in Literacy and Numeracy/GCSE A-C in English and Maths.
Experience:
Experience in a health or social care setting.
Demonstrated ability to work effectively as part of a team.
Skills:
Strong communication (verbal and written) and interpersonal skills.
Ability to adapt to changes and work under own initiative.
Moderate IT skills.
Knowledge:
Understanding of fundamental care needs.
Awareness of current clinical practices.
Knowledge of the role of an unregistered healthcare worker.
Personal Attributes:
Empathetic, supportive, and adaptable.
Ability to Manage:
Effective time management skills.
