Job Description/Overview:
CHOP is seeking a part-time Social Worker to support the Healthy Weight Department. This role (24 hours weekly) involves delivering professional services to patients and families, including psychosocial assessments, care planning, decision-making counseling, social work treatment, and community resource planning. The social worker will collaborate with multidisciplinary teams to provide comprehensive treatment.
Key Responsibilities:
Collaborate with multidisciplinary teams to provide and evaluate treatments.
Facilitate use of individual, public, and private resources for patient and family support.
Adhere to medical-legal reporting and confidentiality requirements.
Manage workload and prioritize based on patient care needs.
Complete documentation following department, hospital, and legal standards.
Identify barriers to effective service delivery and participate in committees to improve social work services.
Follow NASW Code of Ethics and CHOP standards for continuing education.
Qualifications:
Required: Master’s degree in Social Work.
Preferred: Previous experience in a hospital, health organization, or mental health agency.
Licences and Certifications:
Preferred: Licensed Social Worker (LSW) in Pennsylvania.
Preferred: Membership in the Academy of Certified Social Workers (ACSW).
Experience:
Previous experience in healthcare or a mental health agency setting is preferred.
Salary Range:
$55K – $78K annually (Glassdoor estimate).
