Position: Intake Social Worker

Job Description/Overview:
The Intake Social Worker will assess participants’ health and psychological needs for meal service and coordinate service plans. The role involves initiating participant applications, conducting home visits, making referrals, and maintaining follow-up. The position also includes supporting inquiries about home delivery service, collaborating with other case managers, and participating in community affairs related to Meals on Wheels.
Key Responsibilities:
Initiate participant applications and conduct home visits to assess service suitability.
Coordinate and implement service plans, making referrals as needed.
Respond to inquiries about home delivery services and provide assistance to other case managers.
Maintain monthly reports and participate in special projects, community outreach, and in-service training.
Engage in mission-related tasks, such as helping with food distribution or meal preparation.
Qualifications:
Bachelor’s degree in human services or related field.
Experience:
Work experience in human services is required.
Skills:
Proficiency in computer and word processing.
Strong verbal and written communication skills.
Additional Requirements:
Valid New York State driver’s license and access to a vehicle.
Ability to lift at least 35 lbs.
