Job Overview:
Colonial Living & Rehabilitation of Bay City is seeking an experienced Social Services professional to join their team. The role involves providing medically related social services to help residents attain or maintain their physical, mental, and psycho-social well-being.
Key Responsibilities:
Conduct resident admissions, facility tours, and complete admission contracts
Assist residents with transitioning home or to another facility
Serve as Ombudsman for residents
Conduct social service assessments for individualised Care Plans
Prepare quarterly social service progress notes for each resident
Coordinate outpatient appointments and transportation
Assist residents with personal needs, including family contact and weekly shopping trips
Attend resident council meetings, care plan meetings, and other committees
Qualifications:
Master’s degree in Social Work
Two years of experience working in geriatrics
Valid driver’s licence
Licensure preferred
Previous experience with nursing homes preferred
