Job Description/Overview:
CoxHealth is seeking a Social Worker to provide supportive advocacy to patients and their families as part of an interdisciplinary healthcare team. The Social Worker will assist patients in overcoming barriers to their progression of care and facilitate safe and coordinated discharges to the community. Services may include addressing parenting issues, substance abuse, stress management, adjustment to illness, trauma and bereavement, geriatric depression, and other mental health or emotional difficulties.
Key Responsibilities:
Provide advocacy and support to patients and their families.
Facilitate care progression and coordinated discharges.
Address various emotional and mental health issues that may impede patient care.
Communicate effectively with patients, families, and healthcare teams.
Document processes and procedures accurately.
Qualifications:
Education: Master’s Degree in Social Work (Required).
Experience: No prior experience required; recent healthcare experience preferred.
Skills:
Strong communication skills (written and oral).
Analytical and problem-solving abilities.
Organisational skills in a changing environment.
Initiative and flexibility.
Proficient in computer applications, including Microsoft programs.
Licensure/Certification/Registration:
Preferred: Licence in Social Work.
Eligible to sit for certification as a Certified Case Manager (CCM) or Accredited Care Manager (ACM).
