Job Description/Overview:
VIA Health Partners, a community-based, not-for-profit hospice and palliative care provider, is seeking an Admissions Registered Nurse (RN). The RN will present hospice services at the point of admission, initiate individualized care plans, and collaborate with interdisciplinary teams to ensure comprehensive care for patients and their families.
Key Responsibilities:
Coordinate and conduct admission nursing assessments.
Develop individualized plans of care incorporating therapeutic, preventive, and palliative nursing actions.
Communicate with healthcare providers, families, and the interdisciplinary team.
Participate in admission interdisciplinary group conferences.
Provide patient and caregiver education at admission.
Document all clinical information accurately and in a timely manner.
Coordinate nursing assistant care plans as needed.
Maintain professional and productive relationships with external agencies and providers.
Qualifications:
Diploma or Associate Degree in Nursing required; Bachelor’s Degree preferred.
Current, active, and unrestricted NC/SC RN license.
CPR certification required; CHPN preferred.
Minimum of 2 years of experience in direct care of patients with life-limiting illnesses (hospice, palliative care, geriatrics, or oncology).
Experience:
Expertise in managing life-limiting illnesses and end-of-life care.
Experience in hospice or palliative care is strongly preferred.
Skills:
Strong interpersonal and communication skills.
Proficiency in clinical documentation and use of healthcare technology.
Ability to assess and address patient and caregiver needs effectively.
