Job Description/Overview

 

An exciting opportunity within the Manchester Macmillan Palliative Support Service. The Assistant Practitioner will work as part of the Macmillan Specialist Palliative Care Team, contributing to the delivery of a coordinated service for patients receiving palliative care. The role includes supporting patients with life-limiting conditions in maximizing their health and facilitating care in their preferred setting.

 

Key Responsibilities

 

Work within a multi-disciplinary team to provide effective palliative care and support rehabilitation for patients with cancer.

 

Assist in implementing, delivering, and evaluating rehabilitation and education programs to enhance patients’ independence, health, and quality of life.

 

Manage a small caseload under clinician supervision and support patients to enable care in their preferred location.

 

Must be capable of mobility within the community.

 

Qualifications

 

Essential: Foundation degree or equivalent qualification.

 

Desirable: Health-related qualification.

 

Experience

 

Essential: Experience working in a rehabilitation or community setting.

 

Desirable: Experience in palliative care or complex care situations.

 

Skills

 

Essential: Evidence of strong communication skills and ability to work as part of a team.

 

Desirable: Completion of a communication course.
