Job Description/Overview
This role is responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience. The position works in collaboration with the Physician, Utilisation Manager, Medical Social Worker, and bedside RN to ensure timely progression and transition of patients to prevent unnecessary admissions or readmissions. The Care Management process involves communication and effective resource coordination across the continuum.
Key Responsibilities
– Review initial physician admission care plans and assess medical, psychosocial, and financial information from various sources.
– Develop cost-effective care coordination plans.
– Ensure proper documentation to support level of care (LOC) requirements.
– Collaborate with financial counsellors and handle inpatient stay denials.
– Formulate transition plans with input from the health care team, patients, and family members.
– Proactively identify barriers to care progression.
– Address complex clinical and social situations to avoid unnecessary admissions.
– Ensure compliance with regulatory requirements for transitions to post-acute care.
Qualifications
– CA RN License
– BLS certification
