Job Description/Overview:
The Home Care Nurse will provide nursing care in a home care setting, visiting patients in private residences and educational institutions. This role involves monitoring patients’ conditions, administering treatments, managing home care cases, and collaborating with healthcare teams. The nurse will also handle paediatric care responsibilities and support patient education and health promotion programs.
Key Responsibilities:
– Monitor and report changes in patients’ health conditions.
– Assess patients and identify appropriate nursing interventions.
– Administer medications and treatments as prescribed by a physician.
– Provide nursing care and manage home care cases.
– Collaborate with an interdisciplinary health team to plan and evaluate patient care.
– Offer supportive counselling, life skills programming, and health education.
– Conduct disease screenings and perform administrative tasks as required.
Qualifications:
– Bachelor’s degree in Nursing.
– Certifications: Intravenous (IV) Therapy, Tracheostomy Care, Advanced Cardiac Life Support (ACLS), Basic Cardiac Life Support (BCLS), CPR, First Aid.
– Eligible for licensure as a Registered Nurse by the province/territory of work.
Experience:
3 to 5 years of experience in nursing, with specialisation in areas such as Emergency Room, Intensive Care, Palliative Care, and Pediatrics.
Skills:
– Excellent communication and interpersonal skills.
– Strong organisational and time management abilities.
– Ability to work under pressure and attention to detail.
– Flexibility, reliability, and client-focused approach.
