This unique role is an opportunity to build relationships with families and children, and with diverse acute and community services, to anticipate and reduce the challenges that children and young people with complex chronic health needs, and their families, can face. It is an opportunity to ensure the highest quality of care, information and support is provided when families and children need it, so that they can navigate the complexities of their care. This includes a focus on providing care outside the acute setting with outreach to families in the ACT community.
Job Description/Overview:
- The Paediatric Liaison and Navigation Service (PLaNS) has been established to:
- Improve the coordination of care and outcomes for children with chronic complex health care needs.
- Support families to navigate the complexities of shared care with interstate hospital and health services.
