Job Description/Overview:
The Community Mental Health Nurse will manage a caseload of clients with complex needs in the Dementia and Older Persons Mental Health Service. The role includes conducting clinical assessments, risk screening, and providing specialist interventions. Participation in MDT meetings and supporting junior staff and students are also required.
Key Responsibilities:
– Manage a team caseload using CPA principles
– Hold a case-load as Lead practitioner
– Support the duty rota
– Conduct risk assessments and management planning
– Deliver individual and group interventions
– Contribute to the service’s development
– Supervise junior staff and students
Qualifications:
– Registered Nurse with current registration (Mental Health/LD/Adult)
– Completion of Mentorship Course/ENB equivalent
– Continuous professional development
Experience:
– Post-qualifying experience in relevant care group
– Experience in clinical assessments and partnership working
Skills:
– Strong understanding of clinical governance
– Experience with the CPA process

– Salary range: £28,000 – £35,000 annually
